Documentary Requirements

1. Barangay/Business Permit/License for the Current Year
2. Articles of Incorporation/ Partnership, By-laws, if applicable
3. If loan is P1M and above, latest Income Tax Returns
4. If loan is P1M and above, Audited (In-House) Financial Statements (last 2 years), and if below P1M, in-house FS (last 2 years)
5. Contracts with customers/exporters for loans P1M and above
6. List of trade references (contact persons, addresses, tel. no. of at least 3 suppliers and customers)
7. Proof of income such as bank statements, certificate of employment, pay slip, invoices, billing statements, etc., whichever is applicable depending on source of income
8. Photocopy of any government issued Identification Card
