• Current Business Permit
• DTI or SEC Business Registration Certificate
• DTI or SEC Articles of Incorporation, Partnership / By-Laws (for Partnership & Corporation)
• Latest Income Tax Returns
• Audited (In-house) Financial Statements (LatestYear)
• Copy of existing Contracts with customers (if any)
• List of trade references; suppliers; customers (with address and contact numbers)
• 1 valid ID aside from TIN and SMTCDC ID
• Proof of Billing (3 months)
